Print off inspection form to take to a local mechanic
If you plan on completing your vehicle inspection at a local mechanic such as Pep Boys or Jiffy Lube, you’ll need to print off the specific inspection form for your state and take it to their facility, so that a mechanic can fill it out while inspecting your car. Every vehicle inspection form in the US is included below.
Keep in mind that the vehicle inspection is required every year, or 50,000 miles, whichever comes first. You will also need an inspection if you are adding a new vehicle to your profile.
Vehicle inspection form for Uber & Lyft drivers
There are two aspects to getting approved as an Uber or Lyft driver – first, you must be approved as a driver, and second, your vehicle must be approved for the platform, which includes passing a vehicle inspection. Some of the requirements are generic, and others are state specific. Let’s look at each of these requirements:
- 21+ year old driver (18+ for Uber Eats)
- Pass a background check
- Have at least one year driving experience in the US, or at least three years if you are under 23 years old
- Have a social security number
- Have a driver’s license for the state in which you will drive (there are exceptions in some states, especially for military drivers)
- 4 doors
- 5-8 seats and seatbelts (including driver’s)
- Valid insurance with your name as a listed driver on the insurance form
- In-state registration (the registration form does not have to have your name on it)
- Pass a vehicle inspection
- Cannot be a salvaged title
In addition to the requirements listed above which are universal for all states, some states have their own specific requirements. For example, the car cannot be older than 15 years old, but some states require much newer cars. Some states require that your car have no commercial branding on the car, and others may require that you obtain a specific license such as a TNC or business license. Regarding the vehicle inspection, the exact items that are required to be inspected can vary depending on the state, but the minimum items inspected in every state are:
Regarding the vehicle inspection, the exact items that are required to be inspected can vary depending on the state, but here are the 19 points of a vehicle inspection that are required in all states:
- Headlights
- Tail Lights
- Turn Indicator Lights
- Stop Lights
- Foot Brakes
- Emergency/Parking Brake
- Steering Mechanism
- Windshield
- Safety Belts for Driver and Passenger
- Rear Window and other glass
- Windshield wipers
- Front Seat Adjustment
- Doors (open/close/lock)
- Horn
- Speedometer
- Bumpers
- Muffler and Exhaust system
- Tires, including tread depth
- Interior and Exterior rear view mirrors
Some states require a 22-point, 25-point, or even more thorough inspection. One thing to note is that the goal of the inspection is to ensure that the car is decent enough to transport people from point A to point B. The inspection is not intended to check every item on a car. There are mechanics out there that complete 200-point vehicle inspections; these inspections are common for when you want to purchase a pre-owned vehicle, and they take a considerable amount of time to complete. The state-mandated rideshare inspection does not require that level of thoroughness. Rideshare Mechanic can complete your basic rideshare inspection in under 15 minutes, from anywhere your car is, which makes it fast and convenient.

If you plan on taking your car to a local mechanic, be sure to print off the form before you go, as the mechanic will not have the forms with them. Yes, this means that you actually have to use a printer, which many of you reading this probably haven’t used in a while! If you don’t have a printer, you can go to a local office store such as Staples or Office Depot to print the inspection form. Feel free to give the print center this webpage so they can easily download the correct form to print off.
